PARP inhibisyonu
En az 6 PARP geni tanımlanmıştır. p53 gibi PARP-1’in de ikili etkisi vardır. Bir taraftan apoptoza neden olur. Diğer taraftan DNA tamirinde rol oynar. PARP-1 inhibisyonu, bir taraftan DNA hasarı nedeniyle hücre ölümüne neden olurken, diğer taraftan hücre ömrünü uzatır. Bu nedenle, PARP-1 inhibitörleri, duruma göre, hem aşırı apoptozlu hastalıklarda, hem de yetersiz apoptozun eşlik ettiği hastalıklarda kullanılabilir.
PARP-1 inhibitörleri, DNA tamirini azaltma özellikleri nedeniyle, topoizomeraz-1 inhibisyonu ve radyoterapinin etkisini kuvvetlendirmek amacıyla kanser tedavisinde de kullanılabilir.
PARP inhibitörlerinin Parkinson hastalığının tedavisinde de kullanımı uygun olabilir.
PARP inhibitörleri mekanik darbe ve şoklar nedenleriyle oluşan travmatik kafa hasarında nöronal ölümü azaltır. Ayrıca, miyokard infarktüsündeki reperfüzyon hasarlarını da azaltır.
PARP-1’in inhibe edildiği farelerde, kontrol farelere göre septik şokun prognozu daha iyi seyretmiştir. Septik şok bakteriyel lipopolisakkaritlere bir yanıttır. Bu durumda makrofajlar aktive olur. Birçok interlökin eksprese olur ve nitrik oksit sentaz-II (NOS-II) aktive olur. Bu inflamasyon sonucu oluşan reaktif oksijen ürünleri DNA ile reaksiyona girer. Bunun sonucunda endotelde vasküler düz kas hücrelerinin DNA sarmalları kırılır. Bu durum PARP-1’in aşırı aktivitesine neden olur, geniş kanamalarla birlikte endotel hücreleri ölür. Böylece PARP–1 inhibitörleriyle, PARP-1’i baskılanmış fareler kanamalara karşı korunur.
Ayrıca, PARP-1 inhibitörleriyle tip-1 diyabetes mellitusta pankreatik b-hücre harabiyetinin azaltılabileceği düşünülmektedir.[21]
Sonuç olarak, apoptoz ve hücre sağkalımının hücresel mekanizmalarının ortaya konması, kalp hastalıkları, kanser, nörodejeneratif hastalıklar ve AIDS’in tedavisinde yeni tedavi stratejilerinin gelişmesine olanaklar yaratmıştır. Böylece, dejeneratif tıp olanaklarının, kök hücre, progenitor hücre ve somatik hücre nükleer transferi (SCNT) gibi rejeneratif tıbbın getirdiği yeni tedavi olanaklarıyla birlikte kullanılması, geleceğin rasyonel tedavi yöntemleri konusunda yeni ufuklar yaratabilecektir.
